Appointments

Call us at (209) 558-8466 for information and appointments. No prior referral from a supervisor or doctor is required. Requesting appointments for counseling by email is not recommended due to lack of privacy. Employees of Stanislaus County, whose EAP benefits are provided by Workplace Wellness, are entitled to leave time from work for their first EAP appointment. (Supervisor must be informed of employee's absence.) Leave time for additional appointments, which occur during working hours are at the discretion of the supervisor or can be accounted for as a confidential doctor's visit using sick leave.

Forms

When coming to your first session, you will need to fill out simple forms. You can come 10 minutes early to your first appointment and fill them out or you can click here to download the forms and fill them out in advance. *Please note: emailing the forms in is not a secure transaction so we ask that you bring the forms with you.

Confidentiality

All visits with an EAP counselor are strictly confidential, even if the employee has been referred by a supervisor. Information, even to the extent of informing another party as to whether an employee had, or kept, an appointment can be released only with the employee's written permission. It is also important to note that Workplace Wellness does not keep protected health information in a computer system. This means that it is not possible for anyone outside our office to electronically access individual client data.
